While no injuries or accidents have been reported yet, the company recalled 1,100 units because "[t]he front opening between the tray and seat bottom of the high chair can allow a child’s body to pass through and become entrapped at the neck. This poses a strangulation hazard to young children when the child is not harnessed."

You can email firstname.lastname@example.org or call 888-758-5712 for the repair kit, which also includes a DVD that shows you how to install the restraint parts to the tray and how to properly install the crotch strap to the frame.
